Relex HFRA, Human Factors Risk Analysis

Human errors account for an alarming rate of failures throughout a broad range of industries worldwide. With high failure rates and the fact that the results of human error can result in enormously costly failures, including the loss of life, the need for human error analysis and safety assessment is crucial. Methodologies for analysing human error have been developed and adopted in several industries, with NASA being a leader in this effort. Relex Software and NASA have worked jointly to develop a software tool based on the NASA adopted methodology to aid in the analysis of human error and corresponding risk assessment. The resulting Relex ® Human Factors Risk Analysis provides a comprehensive, systematic, and streamlined approach to addressing this critical issue.

Proven methodology. Relex Human Factors Risk Analysis is based on a HF-PFMEA (Human Factors Process Failure Mode and Effects Analysis) approach. HF-PFMEAs can be used to assess human safety and human reliability by analysing human processes or tasks and are a proven approach to reducing the frequency and severity of human error.

 

Risk analytics. Analysis begins with definition of process steps and the possible errors which could occur. The analyst must then determine the effects of those errors, as well as the associated severity and likelihood. Providing a framework for this analysis, Relex Human Factors Risk Analysis computes a Risk Assessment Code (RAC), or risk score, for each error. By colour-coding the RAC results, instant feedback is provided to identify problem areas so that risk reduction techniques can then be effectively employed.

The Relex Architect platform. Relex Human Factors Risk Analysis includes all the advanced features of Relex Architect's robust, proven framework. A rich user interface is coupled with innovative features such as the invaluable Project Navigator for centralised Project control, the Relex Bar for customised one-click operations, the Task List for quick access to commonly used tasks, and data filtering for ultimate user control of data views. Comprehensive reporting capabilities include easy-to-use Report and Graph Wizards for complete user customisation of output reports and graphs. Relex's Import/Export Wizard enables you to transfer your Project data to and from Relex in a variety of popular formats, including Microsoft Excel ® and Access ® . Additionally, Relex Architect modules are built on a common database, for optimal product integration and data sharing capabilities.

 

Built for the enterprise. Relex's Enterprise Edition supports the high performance, scalable needs of large user bases. Features such as support for Microsoft SQL Server ® and Oracle ® databases, permission based security, user and group roles, front end web interfaces, Relex Dashboard for a high-level overview of system metrics, audit trails for tracking data modifications, and customisable data connectors insure that Relex meets the needs of full scale, enterprise-wide teams.

 

Organised framework. Relex Human Factors Risk Analysis assists both the human factors novice and expert in performing a comprehensive and systematic analysis of human errors by providing an organised approach that significantly improves consistency, efficiency, and thoroughness. In addition to being able to enforce the cyclic review of high-risk errors, Relex Human Factors Risk Analysis allows risk scores to be determined and errors to be ranked based on these scores automatically. By cycling through the process of adding barriers and controls and re-evaluating risk scores, Relex Human Factors Risk Analysis offers a streamlined approach to risk assessment and overall risk reduction. Usage of the comprehensive database of suggested barriers and controls enables the analyst to consider various risk reduction techniques. The complete online help system was developed in conjunction with domain experts in human risk analysis to offer guidance during all aspects of the analysis process.
